The undersigned participant, being under the age of 18 years, hereby acknowledges the following:

I desire to use the Park, I have been trained in the use of the OHV and have taken and passed a safety education course regarding operation of the OHV. I understand that the use of OHVs involves a high degree of physical activity and carries a risk of personal injury or property damage. I further recognize there are natural and manmade hazards, environmental conditions, diseases and other risks that in combination with my action(s) or the actions of others can cause injury to me or damage to my personal property. I agree that I will act in a safe and prudent manner to avoid these risks. I also acknowledge that I have familiarized myself with the course and what is required, will wear appropriate safety equipment, including a helmet. I will follow the rules of conduct, will follow the operating procedures and any directions given by an authorized person.

The Undersigned parent of legal guardian of the minor participant (hereinafter “participant”), in consideration of the participants usage of the course does hereby agree to this waiver and release.

I recognize the use of the course involves moderate to strenuous physical activity and may cause physical and/or emotional distress to participants. There may also be associated health risks. I state that the participant is free from any known heart, respiratory or other health problems that could prevent participant from safely participating in any of the activities. I certify that I have medical insurance or otherwise agree to be personally responsible for costs of any emergency or other medical care the participant receives. I agree to release the State of Utah and its agents, departments, officers, employees, agents, and volunteers from the cost of any medical care the participant receives as a result of usage of the course.

I further agree to release the State of Utah, its agencies, departments, officers, employees, agents and all sponsors, officials and staff or volunteers from any and all liability, claims, demands, breach of warranty, negligence, actions and causes of actions whatsoever for any loss, claim, injury, illness, attorney’s fees or harm of any kind or nature to participant arising out of participant’s usage of the facilities. This release to any claim made by parents or guardians or their assigns arising from or in any way connected with the aforementioned activities.

Consent is expressly given, in the event of injury, for any emergency aid, anesthesia and/or operation, if in the opinion of the attending physician, such treatment is necessary. We (the minor and parent/guardian) have carefully read and understood the contents of the foregoing language, have been given a copy of the terms, and specifically intend it to cover participation at the Park sponsored by the Utah Division of Park Recreation.

Informed consent, waiver and release agreement for Jordan River Off Highway Vehicle State Park (Park)

The undersigned, being at least 18 years of age and in consideration of usage of Jordan River Off Highway Vehicle State Park facilities, do hereby agree to this waiver and release.

I recognize the use of off highway vehicles involves a high degree of physical activity and carries a risk of personal injury or property damage. I further recognize there are natural and man-made hazards, environmental conditions, diseases and other risks that in combination with my action(s) or the actions of others can cause injury to me or damage to my property. I do hereby agree to assume all risks that may be associated with or may result from my participation at this park, including, but not limited to actual course activities or while using parking lots, buildings or access paths.

I recognize the use of off-highway vehicles may involve moderate to strenuous physical activity and may cause physical and or emotional distress to participants. I state that I am free from any known heart, respiratory or other health problems that could prevent me from safely participating in any of the activities. I certify that I have medical insurance or otherwise agree to be personally responsible for the costs of any emergency or other medical care that I receive. I agree to release the State of Utah and its agencies, departments, officers, employees, agents and volunteers from the cost of any medical care the participant receives as a result of usage of the course.

I further agree to release the State of Utah, its agencies, departments, officers, employees, agents and all sponsors, officials and staff or volunteers from any and all liability, claims, demands, breach of warranty, negligence, actions and causes of actions whatsoever for any loss, claim, injury, illness, attorney’s fees or harm of any kind or nature to participant arising out of participant’s usage of the facilities. I didn't save my pass. Where is it?

A receipt was sent to the email you gave us. On that receipt, you can use the transaction ID to enter the park.

Can I purchase my pass a day before attending the park?

Yes. Make sure to select the correct date on the calendar provided at selection.

How long is my pass valid for?

A.

Park hours are 6:00 am to 10:00 pm daily (including holidays). At the park’s closing, your pass will expire after the number of days paid for.

Can I buy an annual state parks pass? Where do I pick it up?

No, this option is not yet available. Stay tuned!

Will I get a violation for not having a pass in my window?

A.

No. You don’t need to print out your pass and place it in your window. The rangers will have information about vehicles that have paid and will be able to check on their system.

If I am walking or biking into the park do I need to purchase a pass?

A.

Yes. If pedestrian access is available there will be an option to purchase a pass. Most pedestrian/biking passes cover multiple people.

Can I get a refund if the pass isn’t used?

A.

Unfortunately, we do not offer refunds on this site. More information may be available at the entrance booth of the park.

What if I cannot find my confirmation email?

A.

Check your spam folders. If the email address is invalid, write down your confirmation ID that displays on the page. If you purchased a vehicle pass, the rangers will be able to access your information on site.

How can I ensure my pass doesn’t get lost when I lose cell signal?

There is wifi available at the entrance stations you can use to connect to your email.

Can I pay my camping fees here?
